29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: a) On or about April 24, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications. The Build-A-Box BAB-4 in the east end of the excavation, was not supplied with an adequate number of pins per panel, not backfilled, not within two feet of the bottom of the excavation due to extension of all adjustable legs and missing panels, and had damage to the northeast corner post. b) On or about April 24, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications. The Build-A-Box BAB-6 in the middle of the excavation, was not supplied with an adequate number of pins per panel, not backfilled, not within two feet of the bottom of the excavation due to extension of all adjustable legs and missing panels, and the bottom of the corner posts were not secured to the bottom hole of the extended legs. c) On or about May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications. The Build-A-Box BAB-4 in the east excavation, was not supplied with an adequate number of pins per panel, not backfilled, and was not within two feet of the bottom of the excavation due to extension of all adjustable legs and missing panels and the way it was positioned within the excavation. d) On or about May 2, 2025, at the Foxdale Ave. Water Main Project in Winnetka, employees in excavations were not provided with adequate cave-in protection as the protective system that was being utilized was not in accordance with all specifications. The Build-A-Box BAB-6 in the south end of the excavation, was not supplied with an adequate number of pins per panel, not backfilled, improperly lifted and moved, and the bottom of the corner posts were not secured to all of the extended legs. A. Lamp Concrete Contractors, Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.652(c)(2)(i), which was contained in OSHA inspection number 1605465, citation number 3, item number 1 and was affirmed as a final order on October 25, 2023, with respect to a workplace located at 23rd Avenue & Roosevelt Road in Broadview, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
