29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer failed to ensure that each employee that powered industrial truck operators were competent to operate a powered industrial truck safety, as demonstrated by the successful completion of training prior to operating. This condition exposed employees to struck-by hazards associated with operating a forklift without proper training. T & J Industries, Inc dba Metal Craft Industries was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 1380742, citation number 1, item number 4 and was affirmed as a final order on July 15, 2019, with respect to a workplace located at 1014 South Hillside Drive, Sioux Falls, SD 57110. Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. All training should be documented, including employees name, date of last training and next refresher training date.
