29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. On or about July 19, 2023, at an establishment located at 121 Cedar Street, in Tigerton, Wisconsin; in the Planing/Stacking Building, a quad metallic outlet box, was placed on top of the Mattison Rip Saw, was connected to a flexible cord and used to power a cell phone charger, pedestal fan, and red light guide. The metal outlet box was not installed in accordance with the UL identifier QCIT, which states that boxes shall be mounted in or on a surface exposing employees to electrical shock hazards.
