29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: A) M.L.S. Builders, LLC, conducting work at 19 Winfield Manor Court, Perrysburg, OH 43551: On or about July 1, 2020, the employer did not ensure that the scaffolding platform, supported by the CAT TL642 extending boom forklift, from which the employees were working, was attached and secured to the supporting forks. The employees, working atop the elevated scaffolding platform, were exposed to approximately 11 feet fall hazard from the platform to the ground below. M.L.S. Builders, LLC was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.451(c)(2)(v)), which was contained in OSHA Inspection # 1361781, Citation number 1, Item number 2, and was affirmed as a final order on 03/19/2019, with respect to a workplace located at 460 Cantbury Blvd., Perrysburg, OH 43551.
