29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) On February 11, 2019, an employee was exposed to eye injury and face burns while operating a hot tar heater. Five Star Roofing Systems, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1158842, citation number 1, item number 1b and was affirmed as a final order on August 25, 2016, with respect to a workplace located at 119 N. Tremont, Kewanee, IL 61443. No abatement certification or documentation is required for this item.
