29 CFR 1910.147(e)(3): Lockout or tagout devices removal. Each lockout or tagout device shall be removed from each energy isolating device by the employee who applied the device. On or about 08 May 2017, the employer did not ensure that the keys for Lockout locks were kept in exclusive control of the employee who applied the locks. There were multiple keys for the Lockout locks and keys were hung on a cabinet in the maintenance shop. This exposed employees to potential injuries from unexpected start-up by other employees. Abatement certification is required for this item in accordance with 29 CFR 1910.19(c).
