29 CFR 1910.217(g): The employer did not report within 30 days of the occurrence, to either the Director of the Office of Standards Development OSHA, U.S. Department of Labor, Washington, D.C. 20210 or electronically at http://www.osha.gov/pls/oshaweb/mechanical.htm or the State agency administering a plan approved by the Assistant Secretary of Labor for Occupational Safety and Health. On or about January 19, 2017, at the production facility, an employee's finger was amputated during the operation of a mechanical power press and the employer did not report within 30 days of the occurrence to either the Director of the Office of Standards Development OSHA or the U.S. Department of Labor, Washington, D.C.
