29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: Location: 45 Highland Ave Lowell MA On or about October 5, 2016, the end of the ladder jack scaffold extended more than 18 inches over the support. Luis Quindi dba Speedy Gonsales was previously cited for a violation of this occupational safety and health standard 1926.452(b)(5)(ii), which was contained in OSHA inspection number 992151, citation number 3, item number 1 and was affirmed as a final order on October 24, 2014, with respect to a workplace located at Newell Meadow Condominiums, Chester Rd., Derry, NH 03038. This violation was most recently observed on October 5, 2016 at a jobsite located at 45 Highland Ave, Lowell, MA 01851 where the platform of the ladder jack scaffold extended more than 18 inches over the support. To abate this violation, the employer must ensure that the platforms of ladder jack scaffolds are not more than 18 inches over the support.
