29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: On or about September 1, 2016, employees were exposed to an open floor hole hazard at the northwest corner of the #2090 Erie 2,000 pound steam hammer. The floor hole at the treadle pedal area was 22" long by 12" wide and 6" deep.
