The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) Throughout facility: The employer did not certify in writing the evaluation of experienced powered industrial truck operators when hired who operate the powered industrial vehicles in the facility, such as, but not limited to, a Crown electric pallet jack model WP3035-45 serial 7A296639, a Yale stand up forklift model 5021352-01 and a Nissan sit down forklift, on or about 8/9/16.
