29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a. Site- Employees exposed to a fall hazard are not provided with fall protection training from the employer. The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on July 7, 2016, when employees exposed to a fall hazard are not provided with fall protection training from the employer at 33 and 28 Longfellow Drive, Bethlehem, NH 03574. Ken Stanley and Gwen Lancey DBA G.L. A Plus Exteriors previously committed a serious violation of this standard on October 10, 2014, with respect to a workplace located at 14 Tri City Road, Somersworth, NH 03878 which was contained in OSHA inspection number 1000071. Ken Stanley DBA A+ Roofing previously committed a serious violation of a similar standard on June 12, 2014, with respect to a workplace located at 481 High Street, Somersworth, NH 03878 which was contained in OSHA inspection number 980629. Ken Stanley DBA A+ Roofing previously committed a serious violation of this standard on January 6, 2004, with respect to a workplace located at 338 Route 4, Northwood, NH 03261 which was contained in OSHA inspection number 306944810.
