29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about 10/1/2015, at the Wysong press break; employees were exposed to electrical shock and burn hazards where a relocatable 110V power tap, not designed for industrial use, was used to power a shop-vacuum and a pedestal fan. There was also an unused extension cord attached.
