29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) 211 South Grove Street: On or about March 20, 2015 and at times prior, employees were exposed to becoming entrapped in the excavation in the event it started to collapse while installing a new four inch sewer line without adequate means to egress the excavation. b) 1325 River Street: On or about March 31, 2015 and at times prior, employees were exposed to becoming entrapped in the excavation in the event it started to collapse while installing a new sewer line without adequate means to egress the excavation. Glennco Excavating Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2) which was contained in OSHA inspection number 29486, citation number 01, item number 002 and was affirmed as a final order on September 13, 2011, with respect to a workplace located at 2901 Brooks Street, Missoula, Montana.
