29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about March 18, 2015,while operating the Landis 2.0 Double Long Bolt Threader in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating cutting heads of the bolt threader. b) On or about March 19, 2015, while operating the Number five Warner Swazey Lathe in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating chuck of the lathe. c) On or about March 18, 2015, while operating the Jones/ Lamson 7A Turret Lathe in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating chuck of the lathe d) On or about March 16, 2015, while operating the Tesker Thread Roller number six in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating bar stock as it is fed into the roll threader. e) On or about March 18, 2015, while operating the Tesker Thread Roller number three in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating bar stock as it is fed into the roll threader. f) On or about March 26, 2015, while operating the Horizontal Cantilever Band Saw (left/end) in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded portions of the blade to the left and right of the cutting area. g) On or about March 26, 2015, while operating the Horizontal Cantilever Band Saw (middle) in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded spinning chip auger located to the right of the saw blade. h) On or about March 26, 2015, while operating the Horizontal Cantilever Band Saw (Manual) in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded spinning chip auger located to the right of the saw blade. i) On or about March 18, 2015, while retrieving product out of the SL-25 CNC Lathe, in the Machine Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded chuck and tooling inside the lathe. j) On or about March 3, 2015, while retrieving finished product out of the ZL-45 CNC Lathe, in the Machine Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded chuck and tooling inside the lathe. k) On or about March 18, 2015, while retrieving finished product out of the QT-20 CNC Lathe, in the Machine Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded chuck and tooling inside the lathe. l) On or about February 26, 2015, while operating the Tesker Thread Roller number six in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating bar stock as it is fed into the roll threader. The Dyson Corporation was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 315443630, citation number 1, item number 4 and was affirmed as a final order on 10/27/2011 with respect to a workplace located at 53 Freedom Rd., Painesville, Ohio 44077.
