29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required: a. The employer has failed to submit abatement certification and documentation within ten days of the abatement date, March 13, 2015, for a violation of 29 CFR 1926.100(a), issued as citation 2, item 1, and described as follows: The employer did not require the use of any form of head protection during roofing work. At the worksite located at 4455 Winchester Pike, Columbus, Ohio, employees did not wear head protection while working below employees working on the roof. b. The employer has failed to submit abatement certification and documentation within ten days of the abatement date, March 13, 2015, for a violation of 29 CFR 1926.102(a)(1), issued as citation 2, item 2, and described as follows: The employer does not require the use of any form of eye and face protection equipment during roofing work. This violation was most recently observed at 4455 Winchester Pike, Columbus Ohio. The employees did not use eye protection while using air actuated nail guns. c. The employer has failed to submit abatement certification and documentation within ten days of the abatement date, March 13, 2015, for a violation of 29 CFR 1926.501(b)(13), issued as citation 2, item 3, and described as follows: The employer does not require the use of any form of fall protection during roofing work. This violation was most recently observed at a worksite located at 4455 Winchester Pike, Columbus Ohio, 43232. Two employees were conducting roofing work on a 2 story 8/12 pitch roof without adequate fall protection.
