29 CFR 1926.502(k)(9): The fall protection plan did not include a statement which provides the name or other method of identification for each employee who is designated to work in controlled access zones. No other employees may enter controlled access zones. Location: site On or about 8/17/15, two employees installing roof trusses and working in a controlled access zone listed under the fall protection plan were not included in the fall protection plan. Contractors Imperial Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.502(k)(9) which was contained in OSHA inspection number 1014607, citation number 1, item number 1f and was affirmed as a final order on 02/06/15 with respect to a workplace located at 1700 W Cleveland Street, Tampa, FL.
