29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) A & D Wood Products, Inc., Elida, Ohio: On or about August 26, 2014, the employer did not ensure that both authorized employees engaged in blade changing activities on the Baker Products Baker Band Resaw affixed a personal lock to either an energy isolating device or a group lockout device. A & D Wood Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 108854, citation number 1, item number 3 and was affirmed as a final order on April 3, 2012, with respect to a workplace located at 112 Ward St., Elida, Ohio.
