29 CFR 1910.219(f)(3): All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above floor or platform: a) HBD/Thermoid, Inc. - Bellefontaine, Ohio: The employer does not ensure the chain and sprocket drive on Line #8 Wigwag is enclosed in such a manner as to prevent employees' hands from contacting the ingoing nip point of the traversing chain and sprocket. Employees performing tasks such as guiding and inspecting hose in proximity of the unenclosed chain and sprocket, which are positioned approximately 5 feet above the worksurface, are exposed to caught in hazards. This was most recently identified on or about September 27, 2023. b) HBD/Thermoid, Inc. - Bellefontaine, Ohio: The employer does not ensure the chain and sprocket drive on Line #2 Wigwag is enclosed in such a manner as to prevent employees' hands from contacting the ingoing nip point of the traversing chain and sprocket. Employees performing tasks such as guiding and inspecting hose in proximity of the unenclosed chain and sprocket, which are positioned approximately 5 feet above the worksurface, are exposed to caught in hazards. Employees performing tasks such as guiding and inspecting hose in proximity of the unguarded chain and sprocket, which are positioned approximately 5 feet above the worksurface, are exposed to caught in hazards. This was most recently identified on or about June 28, 2023. HBD/Thermoid, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.219(f)(3), which is contained in OSHA inspection number 1622498, citation number 01, item number 05, and was affirmed as a final order on March 24, 2023, with respect to a workplace located at 1301 W. Sandusky St. in Bellefontaine, Ohio 43311.
