Location: 25 Sixth Street, Chelsea, MA; On or about 03/21/2023, the 2010 Tadano GR800XL (serial number: 547134) was not assembled to meet the manufacturer procedures and instructions which called for the mandatory attachment of the 17,400lb counterweight prior to operation.
