29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Main Shop: On or about August 30, 2017, and at times prior, workers were exposed to struck-by hazards while operating a Ryobi bench grinder without tongue guards to reduce the upper opening to one-quarter inch or less from the periphery of each grinding wheel. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet). West Side Fabrication LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(b)(9), which was contained in OSHA inspection number 1134089, citation number 02, item number 001b, and was affirmed as a final order on 5/16/16, with respect to a workplace located at 2120 Naegele Rd., Colorado Springs, CO 80904.
