Located at Marshalls of MA, Inc., Decatur, GA, USA: On or about August 1, 2017, the employer did not correctly complete an OSHA Form 301 or equivalent for the following workplace injuries and illnesses that were recorded on the OSHA 300 log for 2017. a. On or about 7/13/2017, Ticketer - The employee tripped while walking over rails injuring left wrist resulting in 11 days of job transfer/restriction. The injury/illness was missing on the form. b. On or about 7/19/2017, Ticketer - The employee tripped while walking in congested area injuring left leg and right face resulting in two days away from work. The injury/illness was missing on the form. No abatement certification or documentation is required for this item.
