29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about May 1, 2015, the employer failed to ensure employees changing guides on the Sutton 3-2 Roll Straightener affixed a lock to energy isolating devices for electrical, hydraulic, rolling stock, and pneumatic power sources. The TimkenSteel Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(i), which was contained in OSHA inspection number 971450, citation number 01, item number 004B and was affirmed as a final order on August 7, 2014, with respect to a workplace located at 4511 Faircrest Ave. SW, Canton, OH 44706.
