29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about September 12, 2023, at an establishment located at 5215 Mill Road in Laona, Wisconsin; in Plant B, a Caterpillar 60, Model: GP30, 7AM04119, with fork extensions was not approved by the manufacturer, which affected the capacity and safe operations, and did not have the capacity and operation plate updated to adjust the lifting capacity exposing employees to tip over hazards.
