Location: 25 Sixth Street, Chelsea, MA; On or about 03/21/2023, employer did not enforce standards by allowing crane operator to operate the Tadano GR800XL (serial number: 547134) without the counterweight in place, and has no written work rules in their safety and health program that cover cranes.
