Text For Citation: 03 Item/Group: 001A Hazard:
29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about March 6th, 2020, and at times prior thereto, employees operated a Toyota 7FGU30 forklift equipped with a Caldwell Lif-Truc PB-40 pivoting boom attachment without the manufacturer's written approval, exposing employees to the hazards of tipping over or being ejected from a forklift. b) On or about March 6th, 2020, and at times prior thereto, employees operated a Nissan MAP1F2A25LV forklift equipped with a Caldwell Lif-Truc PB-40 pivoting boom attachment without the manufacturer's written approval, exposing employees to the hazards of tipping over or being ejected from a forklift. C & C Sage, Inc., DBA True Blue Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.178(a)(4)), which was contained in OSHA inspection number 1413192, citation number 1, item number 2a, and was affirmed as a final order on January 23rd, 2020, with respect to a workplace located at 3626 Binz-Engleman Rd., San Antonio, TX 78219.
