29 CFR 1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: Where run through holes in walls, ceilings, or floors: Wires exposed or improperly repaired; unmarked or mislabeled panels, disconnects and breakers; uncovered parts operating at 50+ volts; unclosed openings; missing covers; impermissible use of flexible cords; absence of cord strain relief; improperly sealed junction boxes; corroded disconnects; out lets with blocked ground prongs; absent grounding pins; improperly mounted panel boards; enclosures not installed to exclude moisture; flexible cords improperly repaired; uncovered receptacles; insufficiently accessible panels and disconnects; inadequate training and use of safe work practices; impermissible use of damaged equipment; improper use of equipment in hazardous areas; inadequate hazard warnings; unused openings and spaces inside panels.
