29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Throughout facility; Temporary employees operated powered industrial vehicles, such as but not limited to, the Allis Chalmers LP forklift, the Hyster 80, and the 70 Komatsu, and were not trained on safe operation including classroom training and hands on training under observation. Koser Iron Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 829781, citation number 1, item 3a and was affirmed as final order on June 11,2013, with respect to a workplace located at 150 North 18th Street in Barron, WI.
