29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: Worksite located at 20505 Taylor Street, Elkhorn, NE: The employer is failing to ensure that a platform supported by a fork lift is attached to the forks. There are two employees using the platform who are exposed to falls of 12.5 feet. Affordable Exteriors Inc. was previously cited for a violation of this occupation safety and health standard or its equivalent standard 1926.451(c)(2)(v), which was contained in OSHA Inspection number 906655, Citation Number 1, Item Number 1 and was affirmed as a final order on October 11, 2013, with respect to a workplace located at 176th Street and Patrick Avenue, Omaha, NE, 68116. Abatement certification is required for this violation. The abatement certification sheet is enclosed for the citations.
