29 CFR 1926.651(b)(3): When approaching the estimated location of underground utilities, the employees did not use safe and acceptable means to locate those utilities to determine the exact location resulting in excavator breaking an active underground sewer line in that employees did not safely expose sewer line using shovels: a) Intersection of Churchill Avenue & Berry Street, Somerset, NJ 08873. Condition occurred on or about 07/21/14. Reivax Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(b)(3), which was contained in OSHA inspection number 312861560, citation number 1, item number 1 and was affirmed as a final order on 05/07/2010, with respect to a workplace located at 211 West Prospect Street, Waldwick, NJ 07643.
