29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the PHA team's findings and recommendations: a) PC West 1205 Building: The employer did not develop a system to promptly address the findings and recommendations from the 2009 PHA. Several items, such as, but not limited to those listed below, were found to still be pending, on or about 5/19/14: · Create a PM to inspect or replace check valves annually to ensure adequate backflow (page 2 system 1 DW49060) · Check that the PRD is adequate for this case (page 3 system 1 DW49060) · Determine how the system will be pressure tested before starting up from maintenance activities or other activities where the equipment was opened (page 7 system 1 DW49060) · The interlock action is not accurate per OP No. 96 SB 166 Interlock Test Procedures. The interlock does not block the C 390 vent to SB 166. (page 2 appendix 14 SOL/SOC documentation) · The interlock action is not accurate for 1822PC interlock action (page 2 appendix 14 SOL/SOC documentation) NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.
