29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: a) Jobsite located at 105 N Anderson Dr. Hwy 80 E, Swainsboro GA - A workplace fatality occurred on September 20, 2013, the employee was pronounced at 12:08 pm on the same day; however, the employer failed to report an employee death within 8 hours as required by the standard. The Occupational Safety and Health Administration (OSHA), received notice through the Regional Area Office hotline system at 12:15 pm from a third party on September 21, 2013.
