3389(a). Life Rings and Personal Flotation Devices. At least one U.S. Coast Guard approved 30-inch life ring with not less than 90 feet of 600 pound capacity line attached shall be kept in a conveniently accessible place where employees work exposes them to the hazard of drowning or each employee so exposed shall wear a U.S. Coast Guard approved personal flotation device. On the platform adjacent to water tank 496, the employer failed to provide at least one U.S Coast Guard approved 30-inch life ring in a conveniently accessible place. On 4/29/09, an employee accidentally fell from the platform and drowned in tank 496. The employee was not wearing a U.S. Coast Guard approved personal flotation device.on
