29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On and before 02/09/2012, the point of operation guards installed on the following machinery, located in the Stick Plant, did not prevent the operator from having any part of their body in the danger zone during the operating cycle: a. The box leveler area on the twelve (12) auto pack machines; b. The funnel area on the twelve(12) auto pack machines; U.S. COTTON, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR1910.212 (a) (1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315200428, CITATION NUMBER 1, ITEM NUMBER 02, AND WAS AFFRIMED AS A FINAL ORDER ON 06/30/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 15501 INDUSTRIAL PARKWAY, CLEVELAND, OH 44135.
