Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: ...the hazards of being seriously injured due to being struck by flying drilling mud and its associated high pressure piping from failed connections in the mud supply line running from the Triservice Manufacturing model TSM 500 mud pump to the number 1 drilling rig located at the Lingle number 2 wellsite, located in Section 36, Township 2 South, Range 4 East, Jefferson County IL (near Bluford IL). Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide a positive means of securing the ends of all flexible mudline connections with a safety chain or wire rope during drilling operations, and to insure that such means are properly installed following drilling rig setup and before the mud pump is placed into operation.
