29 CFR 1904.29(b)(3): How quickly must each injury or illness be recorded? You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. a) On or about November 20, 2024, the employer did not ensure that one recordable injury which occurred on November 5, 2024 where an employee received a crushing injury to the right middle finger, which resulted in 1 lost work day was recorded within 7 days on the OSHA 300 log and a OSHA 301 incident report was not filled out. b) On or about November 20, 2024, the employer did not ensure that one recordable injury which occurred on November 8, 2024 where an employee received an amputation to the tip of the left thumb, which resulted in 8 lost work days and a recordable amputation was recorded within 7 days on the OSHA 300 log and a OSHA 301 incident report was not filled out.
