29 CFR 1904.29(b)(1):What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Legacy Flexo - 1327 Glory Road, Green Bay, Wisconsin. On or about June 10, 2024, the employer did not provide the required injury and illness description on the OSHA Form 300 or equivalent for the calendar year 2024. a ) On or about May 12, 2024, while a Supervisor was troubleshooting a press, the employee sustained a cut to the right knuckle while troubleshooting a press (Case log Number 2), which resulted in an other recordable case. The Employer's OSHA 300 Form or equivalent for the calendar year 2024 did not contain a complete description of the object/substance that directly injured or made the person ill. b ) On or about May 16, 2024, while a Press Operator was working with chemicals during cleaning, the employee sustained an eye injury following exposure to ink while cleaning (Case log Number 3), which resulted in an other recordable case. The Employer's OSHA 300 Form or equivalent for the calendar year 2024 did not contain a complete description of the injury or illness and parts of the body affected. c ) On or about June 2, 2024, while a Press Operator was working with chemicals, the employee sustained a throat injury following exposure to ammonia (Case log Number 4), which resulted in an other recordable case.
