29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. At the rebar manufacturing shop, on or about January 17, 2024, the employer exposed employees to a struck-by hazard, in that forklift operators had not been trained and evaluated to ensure that they had the necessary knowledge and skills for the safe operation of the vehicle.
