29 CFR 1910.146(c)(2):The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces by sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language which would satisfy the requirement for a sign. (a)Global Harvest Foods, LLC at 103 South Broadway Avenue, Miller, SD 57362: On or about and at times prior to January 4, 2024, the employer did not ensure that signage or other effective means were used to warn employees of the danger posed by accessible permit required confined spaces on the south side grain storage bins. This condition exposed employees to hazards associated with uninformed entry into a confined space, including asphyxiation and grain engulfment.
