29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) At the Shape Chipper: The employer fails to ensure effective measures to prevent caught-in and struck-by hazards as demonstrated on or about December 18, 2023, where employees engage in maintenance activities such as, but not limited to, clearing a jam. To abate this hazard, the employer must ensure that lockout or tagout devices are affixed to isolate energy to the shape chipper. Beasley Forest Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.147(d)(4)(i), which was contained in OSHA inspection number 1608068, citation number 1, item number 1a, and was affirmed as a final order on 1/17/2023, with respect to a workplace located at 712 Uvalda Hwy Hazlehurst, GA 31539.
