29 CFR 1904.39(a)(2): Reporting fatalities, hospitalizations, amputations, and losses of an eye as a result of work-related incidents to OSHA. Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On or about September 27, 2023, the employer failed to report to OSHA an accident within twenty-four hours that resulted in a hospitalization of an employee. OSHA-Cincinnati Area Office received a phone call from injured employee to let us know about the hospitalization.
