29 CFR 1904.10(a): The employer did not record all cases on the OSHA 300 Log when an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS: a) Production Shop: An employee engaged in polishing granite/stone countertops, utilizing a handheld pneumatic wet stone polisher, was exposed to a continuous noise level at 93.4% of the allowable 8-hour time weighted average sound level (90dBA). The equivalent dBA level of 93.4% is approximately 89.5 dBA. The employees' audiometric test results show that they experienced a work-related standard threshold shift in hearing to the left ear, with a total hearing level of 25 decibels or more above audiometric zero, averaged at 2k, 3k, and 4k hertz, in the left ear. The employer did not record work-related hearing loss on the OSHA 2023 300 log of injury and illness for employees who experienced a work-related standard threshold shift in hearing, on or about, 10/12/23.
