29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Super Hog Pit- On or about September 21, 2023, employees unclogging the Super Hog were exposed to amputation hazards from not utilizing energy control procedures while exposed to an energized woodchipper. b) Flitch Surfacer Area- On or about September 21, 2023, employees operating the flitch surfacer were exposed to amputation hazards from not utilizing energy control procedures while exposed to energized moving parts and 480 volts of electricity when entering into the area of the flitch surfacer. c) Clipping Line- On or about September 21, 2023, a maintenance employee was exposed to amputation hazards from performing maintenance activities on a bundling machine from not utilizing energy control procedures while exposed to energized gears and other moving parts.
