29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wore respirators when such use was not required by the employer: (a) Production Hallway Area: The employer did not provide the information in appendix D to employees that voluntarily used N-95 respirators, on or about 5/18/23 . Nutrastar was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1493816, citation number 1, item number 2c and was affirmed as a final order on 1/8/21, with respect to a workplace located at 700 Central Ave, Farmingdale, NJ 07727.
