29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: 971 Post Rd, Wells, Maine - On or about May 11, 2023, the employer did not ensure employees working on a steep pitched roof at least 18 feet above the lover level inspected fall protection components, specifically ropes, to ensure no wear such as fraying and knots were affecting the ability of the material to sustain a fall. ARJ Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(21), which was contained in OSHA inspection number 1642576, citation 1, item 3 and was affirmed as a final order on March 22, 2023, with respect to a workplace located at 20 Roach St. Quincy, MA.
