29 CFR 1904.39(a)(2): The employer did not report within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: On or about April 5, 2023 and at times prior thereto, where the employer did not make a report to OSHA within 24 hours after the occurrence of an incident. (a) Incident that occurred March 31, 2023. (b) Incident that occurred on April 1, 2023. Dollar General Distribution was previously cited for a violation of this Occupational Safety and Health Standard 1910.39(a)(2) or its equivalent which was contained in OSHA Inspection Number 1568259, Citation 1, Item 1b that was issued on January 28, 2022, with respect to a workplace located at 599 S Main Street, Leachville, AR, 72438. The Final Order Date was February 18, 2022.
