29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a). Production area: On or about 10/27/2022 the employer failed to report two separate incidents that involved two employees sustaining amputations. The incidents happened on 10/27/2022 at 6:52 AM and 10:45 PM respectively. According to documents received, the employer called the OSHA hotline to report the incident on 10/31/2022 at 6:30 PM.
