29 CFR 1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about October 26, 2022, the employer did not ensure that employees who performed service and maintenance tasks on the gear hobbing machines were adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to caught-in and amputation hazards associated with ingoing nip points and rotating parts while performing service and maintenance activities, including, but not limited to, set-up, removing and installing feed and index gears, removing and installing hobbs, shifting hobbs, and lubricating parts. The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 1340573, citation number 1, item number 1 and was affirmed as a final order on September 28, 2018, with respect to a workplace located at 65 Grove St. Prophetstown, IL 61277. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
