29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Roof - An employee accessing a residential porch roof on the front of the house was exposed to a fall of approximately twenty-five (25) feet in that the extension ladder was not extended long enough to allow the ladder side rails to extend at least three (3) feet above the upper lander surface and no grab poles were installed, on or about September 15, 2022. Avila's Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.1053(b)(1), which was contained in OSHA inspection number 1568957, citation number 1, item number 2 and was affirmed as a final order on April 14, 2022, with respect to a workplace located at 1537 West St., Honesdale, PA 18431.
