29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required: a. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for hazards associated with not ensuring that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail. b. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 2a for hazards associated with the employer not assessing the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). c. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 3 for hazards associated with not having an educational program that would familiarize all employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting. d. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 4 for hazards associated with not ensuring that each powered industrial truck operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. e. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 5a-5b for hazards associated with not inspecting cranes for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. And for not completing inspections of the crane at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section. f. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 6a-6b for hazards associated with not providing effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area. g. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 2 Item 1 for hazards associated with not establishing and implementing a written exposure control plan for crystalline silica.
