29 CFR 1910.212(a)(1):Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: a) DOA Line 1; On or about August 24, 2022 and times prior, the employer exposed employees to amputation hazards in that the area between the back of the DOA to the licker-in roller were not guarded when employees were required to monitor the fabrication of thermal insulation. The Bonded Logic, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1537185, citation number 1, item number 2 and was affirmed as a final order on 1/7/2022, with respect to a workplace located at 1465 Shattuck Industrial Blvd La Fayette, GA 30728.
